POSITION SUMMARY:

Responsible to work with residents needing restorative nursing measures to gain or to maintain their highest possible practical functional level.  Responsible for providing consistency between therapists' work and carry over by nursing on a daily basis.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

  • Provides direct restorative care and delegated formalized therapy tasks completing work accurately, safely and in a timely manner, as assigned.Specific care includes passive and active ROM, ambulation, special positioning techniques, splints, assistive feeding devices/adaptive equipment, ADL training, bowel and bladder management, restorative dining, facility feeding program and bladder retaining.
  • Receives specific instructions from formalized therapies when the functions are beyond basic restorative nursing scope and receives follow up support for these tasks from the indicated formalized therapy discipline.
  • Receives specific instruction from nursing on restorative nursing techniques through nursing orders.
  • As appropriate, works with other disciplines in setting up grooming, exercise, sensory stimulation classes, etc.
  • Participates in Restorative Dining Program and works with OT, speech and other nursing staff in accomplishing this.
  • Coaches and assists to make sure residents have their recommended assistive feeding devices in dining room.
  • Coaches and assists regular Nursing Assistants in positioning, ROM, ambulation, ADL, cones/splints/ contracture care, ADL specifics, safety device application and reduction, appropriate padding and wheelchair cushions and safety device release.Conducts inservice for resident restorative care and proper equipment procedures.
  • Follows protocols and procedures for Restorative Care.
  • When directed, attends care planning conference and provides input into development for individualized plan of restorative care.
  • Informs Director of Restorative Services of suggestions for changes that he/she perceives as necessary for improving resident care, that would help a resident care and that would help a resident to reach their highest possible functioning level.
  • Reports lack of functioning equipment and lack or adequate supplies to supervisor.
  • Assists with bowel and/or bladder retraining and works with staff in seeing that the program is properly carried out.
  • Documents progress of residents by completing daily flow sheets to signify the specified restorative nursing measure was performed.Makes weekly observation, progression/regression note in narrative format.
  • Notifies Charge Nurse and Director of Restorative Services when a resident is regressing/progressing or refusing to participate in the program so that an assessment can be performed by licensed nurse(s) and changes can be made in the program and care plan.
  • Assists with orientation and staff development in principles and practices of restorative nursing.
  • Works with unit Nursing Assistants when a resident is transferred from Restorative Care to Nursing Maintenance.
  • Notifies the Charge Nurse and Director of Restorative Services when aware restorative nursing measures are not being consistently done in Nursing Maintenance and resident is regressing.
  • Assists Charge Nurse in keeping lists of restorative approaches current and updated.If these measures are not on the care plan, informs Charge Nurse and Director of Restorative Services so this can be done.
  • Informs Charge Nurse and Director of Restorative Services if an approach is on the care plan is unrealistic so this information can be changed to accurately reflect the resident’s current status.
  • Understands the three levels of rehabilitation and the two phases of a Restorative Nursing Program and assists with interpreting this to the staff.
  • Serves as a resource and teammate to other Nursing Assistants, working with them to provide the specific restorative nursing care for each resident.
  • Works with staff in developing tools appropriate to perform QA audits.Participates in QA audits for Restorative Nursing.
  • Meets routinely with Director of Restorative Services, Nursing Supervisor and Director of Nursing.
  • Reports to and is supervised by Director of Restorative Services.
  • The Rehabilitation Department will provide basic training for Restorative Aides and further training as needed for individual Restorative Care programs.
  • Provides one-on-one assistance and serves as a resource in Restorative Nursing measures to other Nursing Assistants.
  • May be required to perform additional work that follows prescribed or well-established procedures that can be learned within a reasonable time by training on the job.More detailed instructions as to the nature of their duties will be given to employees at the beginning of work.
  • Ability to write a clear and concise narrative progress note.
  • Conscientious, cooperative ability to work as a team member.
  • Ability to speak, write, read and understand English.
  • Ability to cope with the mental and emotional stress of the position.
  • Must function independently and have flexibility and the ability to work effectively with the residents, coworkers, visitors, government agencies and the general public.
  • May have to attend meetings and/or seminars.

QUALIFICATIONS REQUIRED:

  • High school diploma or equivalent.
  • Current listing in good standing in Commonwealth of Pennsylvania Nurse Aide Registry.
  • Knowledge and interest in the care of the geriatric resident.
  • Knowledge and skills in basic Restorative Nursing principles and practices with further training in techniques and practices of Restorative Nursing.
  • Minimum two years’ Clinical Nursing Assistant experience with consistent good evaluations.
  • Must work forty (40) hours per week and work every other weekend.
